The Personal Health Questionnaire/Health Assessment (HQ/A)
HEALTHY YOU offers faculty and staff access to an important health information tool: a personal Health Questionnaire/Health Assessment (HQ/A).
It’s an easy-to-use confidential questionnaire about your health. By responding to this simple questionnaire, you’ll receive valuable information about your health and lifestyle. There are no right or wrong answers, but completing your questionnaire will tell you if you're on the right track to good health and, if not, how to get there.
The questionnaire takes about 15 to 20 minutes to complete and asks basic questions about your health history and your lifestyle. It also includes questions that specifically address health issues for men and for women individually.
Here’s an idea of the types of questions you’ll find:
- How would you describe your overall health?
- How often do you exercise?
- What are your eating and nutrition habits?
- Do you have any medical conditions such as heart problems, asthma, diabetes, or chronic pain?
- Do you smoke?
- How much alcohol do you typically consume?
Plus, you’ll be asked questions about your basic biometric “numbers” such as height, weight, blood pressure, cholesterol, and glucose.
The more honest and thorough you are with your responses, the more meaningful and accurate your results will be.
