Preparing for the HQ/A
The personal Health Questionnaire/Health Assessment (HQ/A) asks for your basic biometric information (height, weight, blood pressure) and the results from a basic blood test (cholesterol and glucose levels). This information is a critical input for the questionnaire. However, we recognize that many faculty and staff members don’t know their personal biometric “numbers.” To help you get this information, most of the institutions participating in HEALTHY YOU will hold a series of “Know Your Numbers” biometric screenings around their campuses. Or, you can get the information from your doctor if you’ve had a physical exam recently, or plan to have one in the near future.
Be sure to have the information on hand when you complete your questionnaire.
