Taking Action on Your HQ/A Results
Once you submit your personal Health Questionnaire/Health Assessment (HQ/A), the system analyzes your answers and produces a personalized action plan with details about your most important health issues. The report will include suggestions for health screenings and provide information about wellness and other programs that may help you improve your health.
You can print a summary of the report to take with you to your next visit with the doctor. Use it to ask your doctor questions and to learn more about your health. With more information, you can take control and, with your doctor’s help, you can make simple changes to improve your health.
Note: If your results indicate you suffer from a chronic illness, such as asthma, coronary artery disease (CAD), congestive heart failure, chronic obstructive pulmonary disease (COPD), or diabetes, you may receive direct outreach from a health professional working with your health plan provider (Harvard Pilgrim Health Care, Blue Cross Blue Shield of Massachusetts, Tufts Health Plan, or UnitedHealthcare).
Your responses to the questionnaire are strictly confidential. Only you and your health plan provider’s professionals are provided with your results. Your employer will not receive this confidential information, and completing the questionnaire will not affect your health plan eligibility or benefit payments.
